That was my I-ROC that cato'd.  As best as I can tell, I suspect I did not have the delay charge completely forward against the O-ring that sits in the bottom of the forward closure and this caused ignition of the ejection charge almost immediately and also ruining my forward closure.  Lesson learned: you can never be too careful! I'm just glad no one was hurt.
